This is the third article in a series about skin cancer. In the first installment we discussed the three main types of skin cancer and the risk factors for their development. In the second piece, we discussed approaches for treating skin cancer, with particular focus on Mohs surgery and how it is the treatment of choice for a large percentage of cancers. In this article, we will discuss issues that revolve around the prevention of skin cancer. It is important to stress that while the cure rate for the majority of skin cancers is high, the best treatment is still prevention. Sun exposure remains the biggest risk factor for the development of almost all cases of skin cancer, and it is something that is largely preventable. While it is impossible to completely avoid sun exposure over the course of your life, understanding how to be smart about sun protection can greatly reduce your chances for having a suspicious growth appear on your skin or minimize the chances of getting a second cancer if you have already experienced a first.

The Basics ? It all adds up
What we have learned over the years is that the risk of developing skin cancer is related to one?s cumulative ultraviolet exposure. In other words, imagine that the skin has a memory ? a running meter of all the ultraviolet light (UV) you have been exposed to, whether from the sun or an artificial source such as a tanning lamp, and this meter starts ticking from the moment we are born. The meter is literally a gauge of how much damage has occurred to the DNA within our skin cells and, the more damage we accrue, the higher the chance for developing skin cancer. So if you want to argue that you only sit in the sun for short spurts or that you never lay out in the sun, the argument doesn?t hold, because your skin doesn?t know if you get your UV light four times for 15 minutes or once for an hour ? it?s all the same (in fact, there is evidence that indicates intermittent sun exposure is worse for you). While there is some inevitable progression of sun damage in everyone?s skin, there are certainly some things to do slow the rate by which it occurs.

Sunscreens
Much confusion revolves around the selection and proper use of sunscreens. With so many products emerging on the market, it is difficult to know which one is right. Typically, most dermatologists will recommend purchasing a sunscreen that is labeled as ?broad spectrum,? meaning that it blocks the two main types of UV light: UV-A and UV-B. However, testing by various consumer agencies tells us that not all broad spectrum sunscreens are created equally and great discrepancies exist between them. In an attempt to demystify the sunscreen situation, the FDA recently announced their first changes to sunscreen labeling regulations in over 30 years, (http://www.fda.gov/forconsumers/consumerupdates/ucm258416.htm). The goal is to create labeling standards to allow consumers to better understand the differences between various products, and all sunscreen companies should incorporate these changes this year. The SPF, or sun protection factor, will still remain. The SPF is a measure of how much longer it will take the average person to burn when the sunscreen is used correctly. So using a sunscreen with an SPF of 15 means that your skin can withstand 15 times the amount of sun before turning red. While it is true that an SPF of 30 filters out about 97% of the sun?s harmful rays, these numbers lose their meaning if the sunscreen is not used correctly. When sunscreens are tested to determine their SPF, they are used at 2 grams per square centimeter of skin. This translates to a great deal more sunscreen than the average person applies, or about a teaspoon of sunscreen for every square inch of exposed skin. In practice, this would mean that you would need one ounce, or about a golfball size of sunscreen to cover your face, neck, arms, and legs applied every two hours. Most sunscreens come as a 3 or 4-ounce tube, so if you are spending a day out on the boat, that would mean using an entire tube within one day. If you are applying less than this amount, than the effective SPF drops precipitously. A simpler way to think about it is if you?re still getting pink or tan while using a sunscreen, the number you are using is not high enough, or you are not applying the product in sufficient amounts.

As 2012 continues, sunscreens will be required to display a product information label similar to labels found on other foods and medications to allow for better comparison by consumers. They will not simply be able to claim ?broad-spectrum coverage? without giving an exact amount of both UV-A and UV-B protection. Those that do not will be labeled as follows: ?Skin Cancer/Skin Aging Alert:? Spending time in the sun increases your risk of skin cancer and early skin aging. This product has been shown only to help prevent sunburn, not skin cancer or early skin aging.?? Other changes that the FDA has mandated include the elimination of outrageously high SPF claims such as 100 or 150, and replacing them with a maximum number of 50+, as well as providing information on what terms such as ?waterproof? mean and providing a time in minutes (either 40 or 80) of how long this protection lasts. Terms like ?sweat proof? and ?water resistant? will no longer be allowed.

The dermatology community as a whole has embraced these changes and sees them as a great way to help consumers better navigate the seemingly endless variety of products and get the best protection for their skin.

The Myth of the Base Tan
As spring is upon us, the lessons learned last summer are by now long forgotten. Instantly, you think before putting that sunscreen on that maybe if you just let your skin get a little base tan at the beginning of the summer or before your vacation, then you can avoid getting those painful pink burns and smearing on gobs of sunscreen. In reality, the idea that a base tan protects you from the sun?s harmful rays could not be farther from the truth. As our skin tans, it is an indication that the DNA within our skin cells is incurring damage. In other words, what signals the body to turn on the tanning response is mutations to DNA caused by UV exposure so, by definition, if you are getting tan, you are causing damage. By allowing skin to stay dark over the course of a vacation or the summer, you are continuously damaging the DNA in your skin and putting yourself at risk for developing potentially fatal skin cancer and premature aging. So for all of you beachgoers who think you are doing yourself a favor by getting that base tan, in reality you are worse off because you have removed your body?s natural warning sign to seek shade ? the burn ? and allowed your DNA to accrue more and more damage, ultimately increasing your chances for developing skin cancer.

But what about my Vitamin D?
The role of vitamin D in our bodies is a topic of recent debate in medicine. We are aware of the important function of vitamin D in bone health and there is some early evidence that it may be important in cancer prevention and a host of other functions. While the exact optimal dose of daily vitamin D is a still a topic of contention, what most physicians and certainly all dermatologists will agree upon is that the sun is not a healthy source to get this dose. It is true that there is production of vitamin D in the skin that occurs with sun exposure, the amount of sun needed to maximize this production is minimal and far below the amount of sun that would make a person?s skin red or tanned. Studies indicate that incidental exposure to the backs of your hands for a few minutes a week is sufficient to maintain adequate levels of vitamin D. Furthermore, common foods such as milk and orange juice are fortified with vitamin D, and vitamin supplements are cheap and readily available. So while the exact role of vitamin D and the correct amount of vitamin D are yet to be determined, what we do know is that UV exposure causes cancer, and anyone who tells you that the sun is a good way to generate vitamin D is doing a great injustice.

It?s Never Too Late to Start
Many skin cancer patients will often wonder if it is too late to start incorporating some of these changes into their lives and the answer is a resounding no. As mentioned earlier, the skin does have a running meter of sun exposure, and there is a point on that meter where the damage is so great that a skin cancer will form. The goal should be to prevent the meter from hitting that point and once you have reached that point at once spot on your body, the goal should be to prevent creating more damage at all the other places of the skin. In other words, the first skin cancer should be a wake-up call to start making life changes with respect to sun exposure in an effort to prevent additional cancers from forming. Broad brimmed hats (not baseball caps or visors), sunscreens, protective clothing, avoiding the sun between the hours of 10 a.m. and 2p.m .

Back in the 50s and 60s, modern furniture was cool. Now people are rediscovering furniture from this period and shopping on eBay for it or buying reproductions of design classics from the past.

The modern style became popular in the UK after the Festival of Britain in 1951. The Festival gave the best of British designers a chance to show off exciting new designs to a public that was hungry for change. In the 50s, people completely reversed their tastes from preferring traditional style to wanting modern design.

Today what was considered cutting edge modern in the 50s and 60s is retro. Retro style is a way to make your home look distinctive, something different from the DIY superstore look that everyone else has.

Re-issues of classic 50s and 60s designs are available at many stores. Habitat, for example, make a number of Robin Day?s classics, such as the Forum leather sofa from 1964.

Fritz Hansen still manufacture Arne Jacobsen?s beautifully organic Egg Chair from 1958.

If you don?t want to pay designer prices, then original designs from the 50s and 60s are still readily available on eBay. Be your own judge of what is good design; not every piece made with a contemporary style in the 50s and 60s was.

Look for simple shapes, such as long, low sideboards with minimal decoration. Generally if it looks good, it is good design.? If the proportions are wrong, ignore it. Scandinavian influence was strong in UK furniture from the 50s to the 70s. Look for pieces finished in teak or rosewood. Rosewood has a distinctive curvy, grain pattern. Sellers on eBay often describe mahogany or sapele pieces incorrectly as rosewood. Both these woods have a straight grain.

They are less fashionable in modern style furniture and consequently pieces in mahogany or sapele are less valuable.

Some manufacturers from the 50s and 60s that made well designed furniture are G-Plan, Stag, Ercol, Austinsuite and Nathan. G-Plan was a pioneer of modern design. Stag employed John and Sylvia Reid to create some fantastic minimalist pieces in the 50s. Their most famous design was Minstrel ? a reinterpretation of eighteenth century elegance for the modern world. Ercol made simple, modern furniture in English Elm. Ercol is of high quality and has a large following. If you want to buy the top designers from the 50s and 60s then pieces do come up on eBay, but are rarely cheap. Look for manufacturers like Gordon Russell, or Archie Shine. Younger also made some marvellous quality pieces in teak and rosewood from the late fifties. They may be cheaper than some of the top designer pieces. Also look for Danish furniture on eBay.

Overall, look for furniture you like rather than what you think is valuable and have fun decorating in a modern style to complement your acquisitions.

Plastic restraints failed to hold pilot, JetBlue CEO vows review

JetBlue CEO says he knew the pilot who become erratic on flight 191 to Las Vegas personally. But JetBlue promises a review of safety procedures.

The CEO of JetBlue said on Wednesday he personally knows the pilot whose erratic behavior led to a diverted flight and that there were no earlier signs of problems, although the airline will review its procedures after the incident.

A JetBlue flight bound for Las Vegas from New York had to make an emergency landing in Amarillo, Texas on Tuesday after what passengers described as chaos mid-flight when Captain Clayton Osbon was locked out of the cockpit.

Aviation experts say that such incidents are rare. John Cox, an aviation safety consultant and former airline pilot, told the Associated Press that incidents in which pilots become mentally incapacitated during a flight are "pretty rare." He said he could only recall two or three other examples in the more than 40 years he has been following commercial aviation.

Passenger video of the situation shows Osbon ranting about Israel and Iran, among other things.

JetBlue CEO Dave Barger described it as a medical situation that turned into a security one. He said he did not think anyone from the company has since talked to Osbon.

"The captain's now in the hands of medical care, obviously, under the custody of the FBI," Barger said told NBC's "Today" program.

"I've known the captain personally for a long period of time and there's been no indication of this at all," Barger said, adding that the pilot was a "consummate professional."

Barger said, "We'll always take a look at procedures ... but I'm very confident about our procedures, the industry's procedures."

He credited the training of another pilot and other cabin crew staff in handling the situation on the flight which had 135 passengers.

Passengers described a chaotic mid-flight scene in which a man in a JetBlue uniform, apparently locked out of the cockpit, began banging on the door and demanding to be let inside.

Tony Antolino, a passenger on the flight who helped subdue the pilot, said the plastic restraints on board were flimsy and systematically failed. Passengers on the flight were rattled, and several had to restrain the pilot after his meltdown by sitting on him.

"They did not work, they failed," Antolino told NBC, speaking from Las Vegas. "We had to use seatbelt extenders, people's belts and physically our hands to try and restrain the guy."

Asked if he would review company procedures following the incident, Barger said: "Absolutely, but not just that, but also the entire event ... within JetBlue and also the industry."

The Federal Aviation Administration cited an "onboard medical emergency" as the reason for the diversion, and said preliminary information showed the co-pilot became concerned that the captain had "exhibited erratic behavior during the flight."

"The captain had exited the cockpit during the flight, after which the co-pilot locked the door," the statement said. "When the captain attempted to enter the locked cockpit,he was subdued by passengers."

The incident was the second one involving erratic behavior by a JetBlue crew member since August 2010, when a flight attendant, Steve Slater, bolted from a plane by deploying and sliding down the inflatable emergency chute following an altercation with a passenger.

Convince a skeptical executive to support social media trainingThis is the second in a series of posts on social media training for employees

A new multi-dimensional training program for all employees probably won?t excite most budget-conscious executives. One that on first glance focuses employee attention on Facebook and blogs is likely to inspire even less enthusiasm. How do you get past those initial objetions to build support for a social media training effort?

There are three fundamental approaches to take in gaining buy-in:

  • Cite internal research
  • Appeal to the leader?s interests
  • Demonstrate the risks in not training employees

Cite internal research

You need to undertake some internal research before you develop a plan or propose anything to your organization?s leadership.

Why, then (you may well ask), am I talking about getting buy-in before internal research? Simple: You may well need to get buy-in for research itself, so building support for your training effort comes in two phases. First, get permission to figure out if training is necessary. Once you get the answer (which in almost all cases will support the case for training) you can make a more informed and relevant pitch to build out the training process.

Among the other information you want to glean from your research are answers to these questions:

  • How engaged are employees in social media? What channels are they using? What do they use it for? How often?
  • Are employees aware of the social media policy? Have they actually read it?
  • How often are employees asked something about their employer in their social communities?
  • How comfortable are employees in answering these questions
  • How inclined are employees to advocate for the company online, from brand ambassadorship to enthusiasm for CSR initiatives?

In the case of PepsiCo (a client with whom I helped develop their training program which just won two?count ?em, two?international awards for its internal social media training program)?the research proved invaluable. According to an Information Week story, ?Sixty-five percent (of employees surveyed) said that friends and family ask them questions about PepsiCo or its products, and more than half said they would like PepsiCo to provide them with information to share across social media channels.?? Yet only about one-third were aware of the company?s existing social media policy.

Numbers like these make it easier to bolster your case for conducting internal training. They also help focus the training on the right kind of material?which is why off-the-shelf employee training just won?t cut it. (How to get good internal research data will be the focus of the next post in this series, by the way.)

Appeal to your leaders? interests

I am routinely surprised by the few number of communicators I talk to who know what their company?s business plan is or what goals the leadership team is accountable for achieving. Having this information in hand can be vital in making a case for social media training.

In fact, it can be vital in making the case for just about anything program you?re working to develop.

Leaders rarely worry about how good the company?s communications are. Their focus is on issues like market share growth, competitive pressures, regulatory compliance and other issues that can make the average employee?s eyes glaze over.

Eking out growth in an uncertain economy topped a 2011 study of CEO concerns, followed by preparing for (and reducing) employee defections and getting a stronger focus on the customer. Number four on the list, however, was ?surrendering to social media.? This was followed by keeping pace with regulations, protecting the company against increased risks, and going global.

If you can demonstrate that informed and trained employees representing the company within their own social communities can positively affect the company?s ability to address these concerns, you?ll build more support for social media.

For example, if a sustainability initiative is important to leadership, knowing that trained employees can help build public support for it can tip the scales. Not only will it help leaders understand how training can help them meet their own objectives; it can help you focus the training so that it not only builds understanding of the policies but also educates and empowers employees about how they can authentically support the effort when talking about the company with the friends and family.

It?s your job to make the business case for social media training, and nothing makes a business case better than demonstrating how it will help the company?s leaders sleep better at night. That?s also a key to the next point:

Articulate the risk of failing to train employees

The kneejerk reaction of a lot of executives to the risks associated with social media is to bolt the doors and lock the windows. It?s a bankrupt approach that does nothing to protect the organization. In fact, a policy that prohibits employees from talking about work not only won?t work, it?s very likely illegal (based on some recent decisions by organizations like the U.S. National Labor Relations Board).

Blocking employee access doesn?t help, either, since most employees have smartphones, tablets or personal networked laptops they can use to bypass the company?s networks. And blocking Facebook can?t stop employees from talking about work with their social communities when they get home.

The best way to mitigate the risks of social media is to make sure employees know their responsibilities and obligations, educate them about the company?s activities, then turn them loose to represent the organization online.

The Altimeter Group?s study on the preparedness of organizations for dealing with social media crises reinforces this idea. According to the study?

Advanced companies do not just dictate guidelines about
disclosure or confidentiality?they empower employees to use social media
professionally. All 18 Advanced companies allow rank-and-file employees to participate in social media as a representative of the brand: five after formal approval, seven within pre-defined guidelines, and six actively encourage participation. For example, IBM ?made a strategic decision to embrace the blogosphere and to encourage IBMers to participate? back in 2003. Coca-Cola allows employees to represent the brand after completing its Social Media Certification Program.

Executives will pay even more attention to Altimeter?s finding that companies that train their employees are less likely to experience a costly or reputation-damaging crisis that erupts in the social media space (regardless of whether it had its origins in social media). According to the study:

Companies with a social media policy, and program to update and reinforce this policy, mitigate the risks by educating employees in advance, on acceptable and unacceptable behavior in social media. Companies with a policy in place are more likely to have employees who know how to safely represent the brand in social media: 62% compared to 23% of companies that did not.

The study also points out that 75 percent of the crises companies do experience were avoidable or could have been minimized had employees been trained to avoid saying the wrong thing, but also trained and empowered to support the right kind of engagement on behalf of the organization.

Presenting your case

Once you have your arguments in hand, you need to present them to leadership in the most compelling way possible. The best approach is to get in front of the top decision-makers to make a formal presentation, just as a product professional would when seeking support for launching a new product line. Leave a summary of key statistics and arguments as a handout. Be sure to categorize your arguments in the simple terms of ?what?s in it for us if we do train? and ?how big the risks are if we don?t.? Draw particular attention to the ways employees engaged in social media can serve as ambassadors for products, services and CSR (and other) initiatives.

Solar storm seen from inside and outside Earth's magnetosphere

ScienceDaily (Mar. 28, 2012) ? For the first time, instrumentation aboard two NASA missions operating from complementary vantage points watched as a powerful solar storm spewed a two million-mile-per-hour stream of charged particles and interacted with the invisible magnetic field surrounding Earth, according to a paper published March 28 in the Journal of Geophysical Research.

The spacecraft, NASA's Two Wide-angle Imaging Neutral-atom Spectrometers (TWINS) and Interstellar Boundary Explorer (IBEX), observed the impact from inside and outside Earth's magnetosphere, respectively. The energetic neutral atom (ENA) cameras aboard each spacecraft enabled global imaging of the magnetosphere, the invisible bubble that protects Earth from the majority of charged particles from the Sun, as it compressed in response to sharply faster solar wind.

The storm, observed April 5, 2010, also is thought to have caused an important communications satellite, Galaxy-15, to founder and drift, taking almost a year to return to its station.

"Many satellites above Earth are in geosynchronous orbit. Like heavy traffic on a Los Angeles freeway, they have to stay in their lanes because they could, theoretically, collide," says Dr. David McComas, assistant vice president of the Space Science and Engineering Division at Southwest Research Institute, and principal investigator of the IBEX and TWINS missions. "More likely, however, is that they will get too close together and their radio frequencies will interfere with the operation of nearby satellites, which could hamper activities on Earth."

The IBEX images (taken from a distance of around 180,000 miles) show an immediate compression of the magnetosphere as it was impacted by charged particles from the solar wind. ENA global imaging enabled the team to determine the precise timing of the compression, to within ?9 seconds. Minutes later, one of the TWINS spacecraft, carrying identical ENA sensors that provide stereoscopic imaging, observed changes in the inner magnetosphere (from a much-closer 28,000 miles). A magnetospheric structure called the "ring current" traps charged particles that gyrate around magnetic field lines. About 15 minutes after impact, the trapped particles propagated down the field lines toward the poles and into Earth's atmosphere, where they produced additional ENAs. The brief time delay in losing particles to the atmosphere suggests that internal magnetospheric processes take some time after compression from the initial impact.

"This imaging gives us a better global picture of the evolution of the magnetosphere -- especially of the processes by which the Sun injects energy into the magnetosphere -- than has ever been available before," says McComas. "It's a cool combination of two data sets that opens the door to much more sophisticated global studies."

To produce the new images, the team also developed a series of techniques to process the imaging data, including improved procedures for binning, differential background subtraction and "statistical smoothing" of images.

"Understanding how solar events develop and impact satellites is like understanding the processes that cause extreme weather events on Earth to develop and destroy homes and businesses," says McComas. "Engineers use weather data to know where and how they need to strengthen buildings against various types of weather threats. The more we know about the processes occurring in space, the better engineers can design satellites to protect them from space weather hazards, which is increasingly important in our highly technological world."

Fearing that climate change could wipe out their entire Pacific archipelago, the leaders of Kiribati are considering an unusual backup plan: moving the populace to Fiji.

Kiribati President Anote Tong told The Associated Press on Friday that his Cabinet this week endorsed a plan to buy nearly 6,000 acres on Fiji's main island, Viti Levu. He said the fertile land, being sold by a church group for about $9.6 million, could be insurance for Kiribati's entire population of 103,000, though he hopes it will never be necessary for everyone to leave.

"We would hope not to put everyone on one piece of land, but if it became absolutely necessary, yes, we could do it," Tong said. "It wouldn't be for me, personally, but would apply more to a younger generation. For them, moving won't be a matter of choice. It's basically going to be a matter of survival."

Kiribati, which straddles the equator near the international date line, has found itself at the leading edge of the debate on climate change because many of its atolls rise just a few feet above sea level.

Tong said some villages have already moved and there have been increasing instances of sea water contaminating the island's underground fresh water, which remains vital for trees and crops. He said changing rainfall, tidal and storm patterns pose as least as much threat as ocean levels, which so far have risen only slightly.

Some scientists have estimated the current level of sea rise in the Pacific at about 2 millimeters (0.1 inches) per year. Many scientists expect that rate to accelerate due to climate change.

Fiji, home to about 850,000 people, is about 1,400 miles south of Kiribati. But just what people there think about potentially providing a home for thousands of their neighbors remains unclear. Tong said he's awaiting full parliamentary approval for the land purchase, which he expects in April, before discussing the plan formally with Fijian officials.

Sharon Smith-Johns, a spokeswoman for the Fijian government, said several agencies are studying Kiribati's plans and the government will release a formal statement next week.

Kiribati, which was known as the Gilbert Islands when it was a British colony, has been an independent nation since 1979.

Tong has been considering other unusual options to combat climate change, including shoring up some Kiribati islands with sea walls and even building a floating island. He said this week that the latter option would likely prove too expensive, but that he hopes reinforcing some islands will ensure that Kiribati continues to exist in some form even in a worst-case scenario.

"We're trying to secure the future of our people," he said. "The international community needs to be addressing this problem more."

Tong said he hopes that the Fiji land will represent just one of several options for relocating people. He pointed out that the land is three times larger than the atoll of Tarawa, currently home to more than half of Kiribati's population.

Although like much of the Pacific, Kiribati is poor ? its annual GDP per person is just $1,600 ? Tong said the country has plenty of foreign reserves to draw from for the land purchase. The money, he said, comes from phosphate mining on the archipelago in the 1970s.
